The following are Dr. Jacobs' current fees per eye. They are subject to change without notice. Please phone for confirmation:

LASIK prices vary widely. Dr. Jacobs' approach is to base his fee on the amount of correction you require, as determined at the time of your consultation. Higher prescriptions require more work and have a higher retreatment rate. We will not lower our fees by providing less postoperative care, since this is a vital part of the process. Nor will we bait you with an extremely low price and then switch you to a higher fee to use a "better" laser. Dr. Jacobs exclusively uses the VISX Star laser. All patients receive the best level of care that we can provide and at a reasonable price. Dr. Jacobs does not require a deposit before scheduling your consultation or procedure.

Dr. Jacobs' laser vision fees include the preoperative consultation, surgeon fee, laser facility fee, manufacturer's royalty plus postoperative office visits and enhancements for one year from the date of your original procedure. Dr. Jacobs will personally see all his patients at each visit.

We are pleased to provide an affordable payment plan which can be tailored to fit your budget. Monthly payments can be as low as $40. Your medical FLEX plan can offer tax advantages.

• 12 months same as cash or up to 4 years at 12.96%
• Minimum amount to finance $1500
• No pre-payment penalties
• Not available for Botox® treatments

Full payment is due the day of your procedure. The following are accepted:
